Job Description

Ecumen Home Care & Hospice-Litchfield seeks a Clinical Supervisor to join its team.  Ecumen Home Care & Hospice-Litchfield provides in-home and community based services in the Minnesota Litchfield area. The Clinical Supervisor is responsible for assisting the Hospice Clinical Director with implementing patient care policies, supervising the direct clinical staff and achieving the objectives of the hospice agency. This position will work directly with the clinical staff to continually upgrade and improve care to the patients. 


Essential job responsibilities include:


  1. Assist in the application of hospice procedures as needed in the field (i.e. nursing visits)
  2. Monitor compliance of departmental and community safety rules.
  3. Promote a customer-centered organizational culture based on the philosophy that the standard of care we provide must be life honoring.
  4. Oversee the interdisciplinary medical record to ensure all phases of documentation and treatments supports effective patient care.
  5. Facilitate admissions of Hospice/Home Health patients with appropriate nursing personnel.
  6. Communicate with Hospice and Home Health nursing team to assure the highest quality of care is being delivered/maintained for each patient.
  7. Assist Hospice and Home Health Clinical Director with other tasks as needed.


Clinical Supervisor

Minimum Required Qualifications:


  • Education: ADN
  • At least one year of management or supervisory experience in hospice or home health
  • Licensure/Certification: RN in Minnesota, CPR and Basic First Aid
  • Current valid driver’s license and proof of vehicle insurance
  • Ability to communicate effectively in both verbal and written formats
  • Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident’s Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations
  • Must possess a thorough understanding of Hospice and Home Health Federal and State Regulations, Care planning, Medicare and other reimbursement systems or the willingness to obtain training in these areas


Preferred Qualifications:


  • Education: BSN and/or CHPN
