Public Health Nurse

Job Description
To provide nursing care, counseling, and education services; to plan, evaluate and coordinate services; and to promote high-level wellness to the citizens of Meeker County.


1.  Assess, plan, organize, and provide nursing and related services; coordinate care with other health team members, human services agencies and other external supports and services; monitor and evaluate progress of individuals and families.
2.  Maintain contact with the physician and other personnel and report significant observations.
3.  Refers individuals/families to other community services and programs.
4.  Maintain complete records of care provided and situations observed. 
5.  Plan, demonstrate, teach and supervise the care which families, students, and para-professionals may assume under the direction of the Public Health Nurse.
6.  Interpret agency services and policies to persons receiving service and other individuals and groups within the community.
7.  Supervision of various programs.
8.  Plan, coordinate and participate in other agency programs, public health clinics, clinics, screening programs, and health promotion and education activities as assigned.
9.  Coordinate programs, services and activities across the department, county and 3 county Community Health Service (CHS).
10. Manage state and federal grants, monitor progress and prepare written progress reports.
11. Participate on CHS Teams as assigned; communicate relevant information back to local agency staff and director.
12.  Actively participate in performance management/quality improvement initiatives  which aim to provide the highest of quality services in the most efficient manner.
13.  Perform similar work as assigned.

  3. The ability to effectively use basic computer software, spread sheets and medical record documentation systems.
  4. The ability to clearly document observations and services.
  5. The ability to oversee programs and grants; prepare work plans, reports, and grant applications for submission to the appropriate agency.
  6. The ability to evaluate progress toward grant goals and outcomes; able to redefine program goals and objectives through a collaborative process with team and community members.
  7. The ability review and evaluate program expenditures and make appropriate adjustments.
  8. The ability to meet timelines.
  9. The ability to lead team meetings.
  1. Bachelors Degree in Nursing.
  2. Current MN RN license.
  3. PH Nursing certificate.
  4. Experience in Public Health Nursing.
  5. Experience in clinical nursing, including acute care.


1.         Knowledge of English, spelling, grammar, vocabulary and medical terminology.
2.         Ability to communicate clearly and effectively with individuals and groups.
3.         Ability to clearly document observations and services and to effectively prepare
            plans, reports, budgets and grant applications.
Knowledge of basic arithmetic.

  1. Ability to sit and stand for intermittent periods of time.
  2. Ability to use computer keyboard for extended periods of time.
